Case Management

Case Managers guide our members through their healthcare journey and ensure they get the care they deserve. Collaborating closely with individuals, they provide guidance on our available services and empower members to actively engage in their own care.

A patient may benefit from Case Management if they have any of the following:

  • More than one of these conditions: Heart Disease, Diabetes, Asthma, or Chronic Obstructive Pulmonary Disease
  • Recently been discharged from an inpatient hospital setting
  • Had a psychiatric admission or if you have been diagnosed with any of these illnesses: Bipolar Disorder, Major Depressive Disorder, Paranoid Disorder, Schizophrenia, or schizoaffective disorder
  • Take 8 or more different medications
  • Struggle with alcohol, opioids, or other recreational drugs

Members who are enrolled in Solis and live in the service area of Miami-Dade, Broward, Palm Beach, Hillsborough, Pinellas, or Polk counties may be eligible to participate.

How can a Case Manager help?

A Case Manager:

  • Works to improve the quality of life, functional status, and overall health
  • Helps navigate the complex care system
  • Helps understand individual health care benefits
  • Serves as an educator when questions arise regarding health care
  • Helps access the community resource you need to live better
  • Supports and reinforces recommended treatments and therapies
