Member Rewards

All Solis Plans include a quarterly allowance offered through the Member Rewards Program. Members can earn up to $400 a year ($100 per quarter) by completing certain health screenings and activities throughout the year. To earn your reward, you must complete these activities on a quarterly basis, with each activity having a reward amount. Using your allowance is fast, easy, & automatic!

Here's how the Program Works

The program operates on an annual basis, starting from January 1,
2024, and runs on a calendar year, ending on Dec. 31, 2024.

  • Q1: January – March

  • Q2: April - June

  • Q3: July - September

  • Q4: October - December

Choose from a series of rewardable activities, with each activity having a dollar amount indicated. Each quarter you are allowed to earn up to $100. Each reward can only be claimed one-time per calendar year, with the exception of Adult Access to Primary Care which can be claimed the first and second half of the year, only 2 times a year.

Mail the signed coupons using the pre-addressed envelope which can be found in your post-enrollment kit.
