Case Managers guide our members through their healthcare journey and ensure they get the care they deserve. Collaborating closely with individuals, they provide guidance on our available services and empower members to actively engage in their own care.
Members who are enrolled in Solis and live in the service area of Miami-Dade, Broward, Palm Beach, Hillsborough, Pinellas, or Polk counties may be eligible to participate.
How can a Case Manager help?
A Case Manager:
Works to improve the quality of life, functional status, and overall health
Helps navigate the complex care system
Helps understand individual health care benefits
Serves as an educator when questions arise regarding health care
Helps access the community resource you need to live better
Supports and reinforces recommended treatments and therapies
